Although the involvement of Team Silent alumni has caused it to be categorized primarily as a horror game. is secretly an action RPG with a time loop plot. The whole game takes place over three days, and players must repeatedly return to pivotal events in the story in order to avert tragedy or recruit new characters.

centers on a Hyoki, a formless, amnesiac spirit that can inhabit other living things' bodies, and Rarities, extra-powerful humans who can maintain a fraction of their free will while possessed. The Hyoki (later nicknamed Night Owl) teams up with a small group of Rarities in order to understand and ultimately exterminate a group of shape-shifting, brain-sucking monsters called Slitterheads, who stalk the dingy, neon-washed streets of the Kowlong Slums. They gradually uncover a Slitterhead conspiracy that cuts to the heart of Kowlong's criminal underbelly, religious movements, and finally, their own lives.
Eventually, Alex discovers that Lisa, a nurse that works at his clinic, is also a Slitterhead. However, she's trickier than any of the others he's faced before, and he's forced to fight her many times over. He eventually gets the upper hand, but the effort leaves him psychologically damaged and perpetually enraged. He redoubles his vow to eliminate every Slitterhead, and tells Night Owl he's taking over.
With the involvement of several Silent Hill alumni, Slitterhead has some great ideas and original features that don’t always come together.
