In 's third Act, the antagonist Orin the Red kidnaps someone you know, taking them as a victim to be sacrificed in the Temple of Bhaal. Rescuing this person becomes one of the side-quests your party can pursue as you try to confront Orin and other loyal cultists to the god of murder. By not following certain objectives, this victim could easily be killed based on your choices.
Once you long rest after arriving in the Lower City for the first time, Orin will appear disguised as the member of the party she has captured. She will only take someone from camp who is not one of your immediate party companions in . However, Orin only abducts the person with the lowest approval of your character from these choices — Lae'Zel, Gale, Halsin, Minthara, or Yenna.
The first step to rescuing Orin's victim is to find out where Bhaal's champion has made their lair in the Lower City, which happens to be a Temple dedicated to the god of murder. A familiar location to players of previous titles, this corrupted location can be difficult to reach. You need to investigate the sewers underneath the city to reach a locked door leading to the place where Orin's followers reside.
This door exists in an area called the Undercity, which you can travel into further by shooting down a hanging corpse bleeding onto a circular stone altar. Doing this unlocks the way forward, where you'll have to face a series of dangerous cultists in Manslaughter Chasm. Defeating these foes will lead you to another locked door, this time blocking the way directly into the Temple of Bhaal.
From here, you can only enter the Temple if you have the Amulet of Bhaal, a powerful artifact hidden somewhere within the Lower City. Much like the rest of Act 3 in , there are many secrets to uncover as you explore the city and interact with various characters. At this point, you may want to consider an offer made by Orin that could ensure your friend's safety before entering the Temple.
