In a similar vein to base , the main story of the DLC has several major decisions which can drastically change the story, which rewards are obtained, and endings for both the DLC's story and the story of the main game. Taking place around the time of the later parts of 's main story, has V delving into a sectioned-off area of Night City known as Dogtown, pulled into a mission to save the president of the NUSA from a crash by the enigmatic Songbird in exchange for a promised cure to V's terminal condition.

Vital to the main story of is the character So Mi, aka Songbird, a powerful netrunner who serves under the command of NUSA president Rosalind Myers. As the first one to contact V about rescuing Rosalind from Dogtown, and about a potential cure for their condition, Songbird is one of the most significant characters in the DLC, and with her subsequent capture by Kurt Hansen, Dogtown's warlord leader, rescuing her serves as the driving force for much of the plot. After making contact with her again, however, it quickly becomes clear that rescuing her — and getting a cure for V — may not be so simple.

One of the major reveals of is that Songbird has been playing both sides — she arranged the forced landing of Space Force One in Dogtown in order to escape serving under Rosalind, and to obtain a cure for a debilitating condition that resulted from being forced to breach the AI-containing Blackwall by Rosalind.
