There are many evil characters in , but some of the villains that are encountered by the player and their party are extremely vile, so much so that they even defy evil playthroughs. While not all of them are exactly antagonists and some can even be recruited, these evil characters are composed of terrible deeds and lines of thought, from crude violence and murder to blatant racism and a desire for domination. Evil playthroughs in are common, with custom characters leaning into less righteous actions.

However, these villains can surprise evil characters by surpassing the standard level of cruelty. As expected from a magical world in which struggles for power are common, there is a large roster of truly repugnant characters in the game, from straightforward villains like Orin the Red to those who are subject to interpretation, such as Minthara in . Nevertheless, their actions throughout the game prove just how evil they are and most of them are deserving of punishment, even from evil custom characters.

Minthara is likely one of the earliest evil characters that players meet in. The Act 1 villain can be sided with and even romanced in the long run, but the prospect of affiliation with her does not make her less vile. Even if manipulated, Minthara is willing to kill everyone in the Emerald Grove, which harbors nature-loving druids and a group of poor refugee tieflings. Showing no sympathy for the weak, the drow Paladin is a bloodthirsty being.
