In an unfortunate snafu, publisher Annapurna Interactive has revealed that physical copies of the PS5 Outer Wilds: Archaeologist Edition have shipped with the wrong game version on the disc, lacking the Echoes of the Eye expansion. Annapurna is still investigating the situation, but players who can provide proof of purchase will be issued a voucher to redeem it as DLC.

For the record, all PS4 owners can upgrade to the PS5 version of the game free of charge, while Echoes of the Eye is a paid expansion (and quite good, too).

I remember the first run of Final Fantasy Tactics Greatest Hits for the PS1 had a bad batch that wouldn't read. It was the first thing I ever ordered online, and we had to return it to an EB Games
