Baldur's Gate 3 just got a new achievement with the latest patch - and it's impossible to acquire. Foehammer, which was just added to the GOG release of the game, mentions beating the main story in "Honour Mode", which is presumably a new difficulty setting even harder than Tactician.
Honour Mode cannot be selected in-game right now, so it's not clear how this will work. However, if it's anything like Honour Mode from Larian's previous game, Divinity: Original Sin 2, then it will likely complement the Tactician difficulty while making it harder to save scum your way throughout Faerûn.
In Divinity: Original Sin 2, Honour Mode gives you just a single save slot, so right off the bat, you wouldn't be able to make a bunch of different saves for the different scenarios in-game.
