It appears as though Doctor Strange chose his successor as Sorcerer Supreme to spite Doctor Doom. Heavy is the head that wears the crown — especially when it comes to sorcery. There's a sense in which Sorcerers Supreme like Doctor Strange are secretly Marvel's darkest heroes — because with their position comes a terrible responsibility. They stand at the forefront of every single magical confrontation, constantly targeted by the most powerful interdimensional entities such as Dormammu and Chthon. Meanwhile, they also have to deal with rivals who desire to supplant them as Sorcerer Supreme. Suffice to say no Sorcerer Supreme ever gets to retire.
Sadly, that's proved true of Doctor Strange. Stephen Strange has finally been killed, but before he died he appointed his successor. His beloved ex-wife Clea is now Sorcerer Supreme of Earth, a rather surprising choice because she already holds that role in another dimension. This twist of fate is made all the stranger because no previous Sorcerer Supreme has ever been appointed by their predecessor; the role is typically conferred by the mystical entities known as the Vishanti, and frequently entails a contest between different mages on Earth or some great act of heroism. In Strange #1, by Jed MacKay and Marcelo Ferreira, there's a subtle hint that Doctor Strange subtly warped the line of Sorcerer Supreme to ensure the mantle could be passed on to his ex-wife.
