And if you opt to keep V alive, it turns out that the damage is done and they're dying anyway. So really, you're just deciding how long V's body can keep going, and who gets given the keys.
With this in mind, it's no wonder that many fans are hoping that the upcoming expansion, Phantom Liberty, gives us a new ending for the main story. While it will be set during 2077's main quest, players still hope that its ramifications will open up another ending for V and Johnny back in the base game, and perhaps one that ends a little better for everyone involved. Or even worse, because this is a dystopia, of course.

"Do you think Phantom Liberty will give us another ending with a different fate?" asks Reddit user TomAnyone in r/cyberpunkgame.
Considering the DLC will see V work with the NUSA government, many do believe it will have an effect on the game's ending. However, they don't necessarily believe this will be for the better.
"Yes but the new ending could be worse than actual death," suggests ReversedSandy. "We know it’s not gonna be a truly good ending no matter what."
Others agree, seeing the NUSA play a sinister role in Cyberpunk 2077's ending. "I think what would happen is NUSA would probably replace Arasaka for a devil ending," says Fallen_Walrus. "I mean does the game ever give us a reason to trust the government? Let alone a government you're not a citizen of and a merc being used by them."
