There is so much to do and see in that it's extremely easy to miss a lot of it. In fact, players with upwards of 1,000 or more hours in the game are still missing some minor and even major features and quests. It's incredibly impressive just how much has been crammed into, so much so that dedicated fans can easily miss out on a lot of it.
One such feature is to do with companions and includes one of the many hidden quests in that players can very easily miss. This is actually quite a fun mission, and it can pop up more than once, depending on how well players tend to their settlements. Surprisingly, this feature has been so well hidden that most players have never even heard of it.
Players can have their companions and other settlers kidnapped in thanks to a Minutemen radiant quest that can occur randomly throughout a playthrough. This quest can be started when players talk to Preston Garvey, listen to Radio Freedom, or talk to any settler. They will inform the player that a settler, who could be one of the player's currently inactive companions, has been kidnapped and is being held for ransom. This was shown by user MisterAtlas_ on Reddit, who had Piper the companion kidnapped.
This is a completely random event, one that isn't prompted by any of the player's actions, and can even occur before fans join the Minutemen. It also isn't the same as losing companions in, which is a different issue altogether. So, it's no surprise that the majority of players haven't encountered this radiant quest, even after sinking thousands of hours into the game.
