Moon Knight episode 4 delivers a major surprise as Marc Spector is shown to be a hospital patient, but he isn't the only familiar face from prior episodes admitted to the facility or working for it. One of the big selling points of Marvel Studios' Phase 4 Disney+ show early on was the promise of exploring mental health and Moon Knight's dissociative identity disorder. Moon Knight has so far explored this by featuring two of Moon Knight's alters, Marc Spector and Steven Grant. Now, the series has changed the context of this relationship and what happened before by making Marc Spector a patient in a psychiatric hospital.
Throughout the first half of Moon Knight season 1, the show has kept its focus relatively small. There are only a handful of connections to the Marvel Cinematic Universe, as the series instead leans into telling its self-contained story. In addition to the dynamic between Marc Spector and Steven Grant, the other main characters in Moon Knight include Marc's wife Layla (May Calamawy), the main villain Arthur Harrow (Ethan Hawke), and Khonshu the Egyptian moon god (F. Murray Abraham). There are additional characters featured in each episode, but most of them have small roles in the grand scheme of the story told thus far.

The ending of Moon Knight episode 4 recontextualizes the appearances of several characters, though.
