All five of them are free, you just need to head through this link to the official PlayStation website, scroll down, and claim a code. You can then enter that into the PS Store and the PSN avatars are yours.
You'll be treated to PSN avatars based on Uncharted: Legacy of Thieves Collection, Ratchet & Clank: Rift Apart, Kena: Bridge of Spirits, Tchia, and Horizon Forbidden West. Here's what they all look like:
