Bloodsail Shirt Bloodsail Pants Bloodsail Sash Bloodsail Boots Bloodsail Admiral's HatThe first four only became cosmetic with Patch 10.0.5, and are a reward from the Stranglethorn Vale quest, Dressing the Part. The hat is a reward for the quest Avast Ye, Admiral!. However, it's not quite as simple as heading to Stranglethorn Vale and picking up these quests. In order to see the quest for the basic uniform, players need to be at least neutral with the Bloodsail Buccaneers faction, and hated with the Booty Bay faction. For the hat, players still need to be hated with Booty Bay, but friendly with the Buccaneers. Becoming the pirates' friend is not only a bit of a grind, it will anger the Goblin towns of Everlook, Gadgetzan, and Ratchett as well as Booty Bay. Thankfully, there is an easy way to at least get neutral with those towns again.
