Warning: Contains SPOILERS for Obi-Wan Kenobi episode 5
Obi-Wan Kenobi episode 5 climaxes in a confrontation between Darth Vader and Reva, the Third Sister, but it's unclear whether the latter actually dies at the end of the episode. During Order 66 and the attack on the Jedi Temple, Anakin Skywalker attacked Reva, leading to her wanting revenge against Darth Vader. This revenge quest comes to a head in Obi-Wan Kenobi episode 5 as Reva attacks Vader.
