Warning: Spoilers for Obi-Wan Kenobi episode 5.
The tragic past of Reva, a.k.a. the Third Sister, is revealed in the fifth episode of the Obi-Wan Kenobi series, which Darth Vader is somehow already aware of. Obi-Wan Kenobi opens with a group of Jedi younglings and their teacher holding off waves of mind-controlled clone troopers during Order 66, and one of the younglings is revealed to be a young Reva. The Third Sister’s motivation is further revealed as well, and it concerns her Sith assailant more than even the legendary Obi-Wan Kenobi.
As revealed in flashbacks and dialogue in the Obi-Wan Kenobi episode “Part V,” Reva and her group of younglings were confronted by Darth Vader himself after the clones killed their master. Vader cut down the younglings, with Reva only surviving by playing dead. Vader hadn’t constructed his red-bladed Sith lightsaber yet, nor had he received his cybernetic survival suit, so Reva recognized him as Anakin Skywalker and plotted her revenge from within the ranks of the Imperial Inquisitors.

“Part V” shows Reva, seeing an opportunity to betray the Sith Lord, attacking Darth Vader on Jabiim, but her efforts fail, leaving her badly wounded and left for dead. As he deals the debilitating lightsaber strike, Darth Vader also reveals that he knows she was a youngling he’d attacked in the Jedi Temple during Order 66. Vader, who is one of the galaxy’s most powerful Force-users in both identities, likely felt a similar presence in the Force when confronting her in the Jedi Temple and as an Inquisitor. Vader’s knowledge also raises other interesting questions, such as when exactly he discovered her secret past and why he only mentioned it
