Radroaches have been prominently featured in the Fallout universe since the release of Fallout Tactics in 2001 and each subsequent title has had the enlarged bug, including the modern Fallout 4 and Fallout 76. One Fallout 4 player has shared their first encounter with an enormous Radroach after playing the game for a whopping 2,000 hours.
Reddit user Jake3245 recently posted a clip in which they're trying to access the Overseer's terminal in the early Fallout 4 location of Vault 111. Given that the Fallout 4 video takes place in the first Vault, Jake3245's clip could have taken place at any moment in their save, though they seem to have some skill points beyond the introductory stats. There are many reasons Fallout 4 players have to return to Vault 111 beyond exploration as it also has hidden loot like the Cryolator and Red Menace minigame for the Pip-Boy and environmental storytelling that may have gone unnoticed during an initial playthrough.

Jake3245 Fallout 4 video starts with the player opening the evacuation tunnel through the Vault Overseer terminal before pulling out a 10mm pistol to fend off any enemies along the way. As the player enters the tunnel, they see one of Fallout 4's anomalies as an enormous Radroach that Jake3245 has dubbed a «Roach of unusual size» is scaling the Vault 111 wall. Jake3245 immediately aims at the large Radroach and takes a shot at it before it jumps from the wall and starts running towards the Fallout 4 player as a normal-sized Radroach would.
