This Cookbook is a difficult one to obtain. You’ll need to defeat Mohg, Lord of Blood in the Subterranean Shunned-Grounds, and then hit the altar behind the chest to reveal a passage behind it.
Follow this path until you come to the top of a huge tower, with rafters spanning it. You can take the stairs here, but to reach the Cookbook you should carefully drop to the next set of rafters, then the following set. The Cookbook is on a corpse at the end of this third set of rafters.
