Inumaki lost his left arm because of Sukuna’s attack during the Shibuya Incident arc.
Inumaki was last seen during Season 2 Episode 13 (Episode 37 overall) where he was helping civilians. Season 2 Episode 23 (Episode 47 overall) shows Yuta Okkotsu meeting with the higher-ups. Yuta tells them determinedly that he will kill Yuji Itadori himself. He tells them this because according to him, it was Yuji who cut off Inumaki’s left arm in Shibuya. After that, the higher-ups officially assign Yuta to become Yuji’s executioner.
This was actually just a strategy so the higher-ups would be convinced and agree to Yuta becoming Yuji’s executioner. Yuta needed some sort of personal motive to show how willing he was to kill Yuji. Eventually, Yuta reveals to Yuji that he’s on Yuji’s side, empathizing with him because he went through a similar experience. At some point, Gojo also told him to take care of the first and second years in case something happened to him.
The manga doesn’t reveal much about what happened to Inumaki following the Shibuya Incident arc. However, we will eventually see that he’s alive and well. When Gojo’s allies meet to discuss what to do in the Culling Game arc, Inumaki is seen in the background. This is in chapter 220 of the manga. He is also there when the group unseals Gojo from the Prison Realm, which happens in chapter 221.
