Carian Piercer's item description reads:
One of the sorceries of the Carian royal family. Conjures a magic greatsword to impale foes. Charging lengthens the initial step in. This sorcery forms a counterpart to Carian Greatsword. Highly lethal when used against single foes riding alone.
Carian Piercer can be obtained by killing a Teardrop Scarab within Caria Manor in northern Liurnia of the Lakes. This area is filled with small and large hand enemies that can pose quite a challenge for under-leveled characters, so proceed with caution.
When entering the manor, head east and navigate behind the gardens to find the Teardrop Scarab. If you don't kill it quickly, it will disappear and force you to reset the area to spawn it again, so be sure to use your strongest melee attacks or magic spells to take it out. When it's dead, you'll net Carian Piercer.
