Grafted Scion is a Field Boss in Elden Ring. It is the first boss you will encounter and a pretty challenging one to beat. You could say that defeating it early on is near impossible because you do not have any healing items yet. One can stick around and beat it, but dying to it once will make you proceed further into the game. However, you can face it later in the game if you want to.

Grafted Scion makes its first appearance in Chapel of Anticipation, which is the game's tutorial area. The boss is optional, and facing it will not affect any ending in the game whatsoever. Defeating it or dying to it on your first attempt will result in seeing the same cutscene and reaching the main path. Defeating the boss will get you the Ornamental Straight Sword and Golden Beast Crest Shield. The sword is a really unique weapon with a very interesting and fun skill. You can return to the boss from the Four Belfries in Liurnia of the Lakes. Defeating the boss early on can be advantageous for you because of the high number of souls you will receive and can use to level up your stats.
If you want to start your NG+ run, then you will find the boss easier as now you have access to multiple Flasks and, most probably, a maxed-out weapon. This will allow you to deal massive damage to the boss in NG+ and make the fight go quicker. Using good Blood weapons will make the boss bleed relatively quicker, making it lose a huge chunk of health.
