Warning: contains spoilers for Hulk #4!
The death of Jean Grey is one of the most tragic moments in X-Men history, and the Marvel Comics universe in general — but Marvel can't stop itself from continually changing the infamous event. Case in point: Hulk #4 shows yet another change to the last moments of Jean Grey before she is brutally killed — but this change is quite radical, not to mention extraordinarily gruesome. Unfortunately, Jean Grey is best known for dying — and Marvel knows it.
Jean Grey was a relatively uninteresting character in the X-Men's early days; her major claim to fame was being a romantic interest for ever male on the team, as Stan Lee wrote Cyclops, Iceman, Beast, Angel (and even Professor Xavier!) as being madly infatuated with their new teammate. Under Chris Claremont, Jean was an interesting character irrespective of her relationships with other men, but her partnership with Cyclops endured. The Dark Phoenix Saga was an effort by Claremont to center a story arc entirely around Jean, and it quickly became a fan favorite.

Jean Grey, calling herself Phoenix, is used and manipulated by Mastermind of the Hellfire Club, turned against her friends, and eventually acquires phenomenal power. To recharge her power, Jean Grey drains a star, which goes supernova and destroys an entire planet (and all the inhabitants on the surface). Following a conflict with the Shi'ar Empire, Jean sacrifices her life rather than allow her powers to take control of her, and the arc is memorably summed up with an infamous image of Cyclops holding her body in a Pietà-esque pose. This notably does not happen in Hulk #4.
