Pyramid Head is one of gaming's most iconic villains, first debuting in Silent Hill 2 way back in 2001 before appearing again in movies and other games like Dead by Daylight. And its creator Masahiro Ito has revealed what inspired him when designing the character - Braveheart.
Braveheart is a Mel Gibson movie from 1995 that told the story of Scottish warrior William Wallace and his fight against Edward the First of England. In one scene, Braveheart is being ferried off to a beheading where several executioners are waiting, one of which will take an axe to his neck. These influenced Ito when it came to making Pyramid Head.

"[After] I designed P-Head based on the 1st image in this tweet, I started thinking the background," Ito said (thanks, IGN). "I remembered the film/that [people] who put a bag on their heads & then I looked the [people] up in the books. I finally came up [with] the idea of an executioner of the old town."
In essence, Braveheart sparked a deep dive for Ito into other films and books where characters placed bags over their heads. He shared another tweet of his from 2017 where he had posted artwork of his from 1993 and 1996 when he was an art student, titled Strange Head. These too served as inspiration for the Silent Hill character.
Pyramid Head appeared in subsequent games but given that Silent Hill has been dormant for some years, their most recent appearances are in crossovers or via merchandise, such as with the aforementioned Dead by Daylight. That being said, film director Christophe Gans has said that there are "several games" in development, so it might not be too long before we dive back into its world.
