Early in his career, one of Spawn's weirdest powers actually showed the face of his replacement years before he was even given a name. When Al Simmons was still grasping at the reality of his new existence as a Hellspawn, he sought to use his powers to fix his disfigured face so he could see his wife, Wanda Blake, again. But for some reason, his efforts had him put on a different face altogether, one that was later retconned to be his future replacement, Jim Downing who operated as Spawn when Al could not. Long before Todd McFarlane created the Spawn multiverse, he hinted at the hero's replacement.
Having returned to Earth after being dead for five years, the former Lt. Colonel Al Simmons appeared in New York in an unfamiliar costume with no idea who he was nor how he got there. After helping to save a woman from being mugged, Simmons learned he had abilities now beyond anything human, abilities he had acquired after making a deal with the devil. He remembered enough that he had agreed to this deal so he could see his wife again. His confusion turned to horror when he removed his mask to reveal a mangled and burnt face unrecognizable from his former handsome visage, his bare hands confirming the rest of his body matched, leaving him angry, confused, and remorseful. Though Spawn's costume and powers might be deadly, all he wanted was to see his wife.

In Spawn #2 by writer and artist Todd McFarlane, Simmons sought to visit his wife but knew that she would not recognize nor want to see him in his current condition.
