On March 12th, 2022, the Il Molise bonus stage was added to Vampire Survivors through an update. This stage is quite different from the other stages. For starters, the player only needs to survive for 15 minutes; normally the player must survive for 30 minutes. The enemies are also very different in Il Molise. Instead of having the enemies charge the player mindlessly, the enemies in Il Molise are unmoving plants and spectral creatures.

Scattered among the enemies are pots that leave a gold coin when broken, making this the best stage for amassing large sums of gold. This stage is also the best stage for players attempting to break their previous kill-count score. Unlocking this stage isn't complicated, but for new players, it might be challenging. Il Molise is by far the easiest of the stages in Vampire Survivors, mainly due to the fact that the enemies don't attack the player unless they get too close.
Put simply, the Il Molise stage is unlocked when the player unlocks the Hyper Mode setting for three other stages. Unlocking the Hyper Mode setting for the three normal stages requires defeating the boss that appears at minute 25.
