The demo for Metal Hellsinger was mostly about showing off the gameplay and is light on story. The player takes on the role of The Unknown, a demon who must tear through the eight hells and their demonic hordes. The Unknown's adventure is narrated by Paz, a disembodied skull who also acts as one of her weapons. Metal Hellsinger is an arena-style shooter, similar to the recent Shadow Warrior 3's style, where The Unknown must slay groups of enemies in enclosed areas before she can proceed.

The main gameplay mechanic of Metal Hellsinger is performing attacks to the beat of the music. The crosshair has a metronome animation that matches the current song playing and performing attacks at the right time causes The Unknown to deal more damage. The Doom inspirations for Metal Hellsinger are obvious from the start, especially as the game shares the execution mechanic of the Bethesda era of the franchise.
