Being able to wield Thor’s hammer was always considered proof of worthiness for those who succeeded, but Moon Knight episode 5 just made Mjolnir's test look tame. First set up by Odin’s (Anthony Hopkins) spell in 2011’s Thor, Mjolnir’s test was a staple across MCU Phases 1-3, presenting itself as both a recurring joke and a somewhat exclusive worthiness test. Throughout the MCU to date, only Captain America (Chris Evans) and Vision (Paul Bettany) were the heroes able to wield Mjolnir apart from Thor (Chris Hemsworth) himself — with both characters' hearts considered "worthy" at the time they lifted Thor's hammer.
Moon Knight episode 1 already introduced the heart as a manifestation of someone’s worthiness for ancient Egyptians, but it was Moon Knight episode 5 that cemented its existence in the MCU. To be deemed worthy of passing through the Field of Reeds, someone’s heart must be weighed against the Feather of Truth on the Scales of Justice and balanced during the underworld journey. As Marc was shot and killed by Harrow at the end of Moon Knight episode 4, both Steven and he found themselves on Taweret’s ship, where the Scales would finally judge their worthiness in life.

Wielding Mjolnir might have been proof of someone’s worthiness, but it never had the implications the heart weighing on the Scales of Justice had. Indeed, only a few were deemed worthy of Mjolnir, but they didn’t suffer any consequences if unable to do so, as proven by most Avengers trying to lift Mjolnir and failing with no harm whatsoever inflicted upon them in Avengers: Age of Ultron.
