Being a Yonko, Big Mom had her own territories, which were out of the reach of everyone including the Marines. They knew that if they tried to invade any of her territories, they would be risking an all-out war with Big Mom. One of Big Mom's territories was Fish-Man Island, but after Luffy's exploits, he took it under his wing. Despite losing Fish-Man Island to Luffy, Big Mom still had Totto Land, which is a massive territory, and Big Mom ruled over it. She oversaw over every single thing that took place in the territory. Big Mom's status as a Yonko prevented any foreign interference and kept the populace safe.
However, Big Mom's reign as one of the Yonko came to an end in the Wano Country arc. In the arc, Big Mom took on the combined team of Eustass Kid and Trafalgar Law. The two Supernovas knew that they needed something special to defeat someone like her, so they used their devil fruits awakenings and after an intense fight, Big Mom rested in a pool of magma. Whether she is dead or will she survive is irrelevant, but she has lost her position in the Yonko. This effectively makes Totto Land a normal island with pirates, and since their main base is Whole Cake Island, the Big Mom Pirates could be in for a world of trouble as it won't be long before the big names in One Piece start making their moves.
